Sick notes

If you’ve been off sick for more than 7 days

To request a sick note:

  • fill out a sick note request form
  • Complete an E-consult online form
  • Please note sicknotes cannot be issued prior to the date they run out. They can only be issued from the day they run out
  • Request for sick notes are rountine and not considered a medical emergency
  • Sick notes can take up to 5 working days to complete
  • You can request your sick note is email to you
  • We do not take sick note requests over the telephone, they must be put in writing as stated above
  • Please be aware that if the GP has not previously consulted with you, your request for a sick note may be rejected 

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
